TMCS CC was founded by Mr. Jeffrey T King, RN, MBA, MSN in an effort to assist individuals and families to navigate the complex healthcare system and access community-based and social support services.  The TMCS Community Care mission is to build bridges to close the gap between individuals or families and the health and social resources they need, ensuring equitable access to community and public services to ultimately improve communities. TMCSCC offers a range of community base care coordination, care transition and case management services as well as direct services including health education, and the iDad fatherhood program.
